Supplementation guidance

This document provides guidance about reviewing supplementation rates as part of the Baby Friendly audit and assessment process.

It includes forms for health services to use to monitor supplementation rates and to review supplements when given as a result of a clinical indication or a maternal request (Supplement details forms), together with an action plan template to help plan any necessary changes in order to improve care. It should be read in conjunction with the supplementation audit which is part of the Baby Friendly audit tool.
